PLEASE CALL FOR AVAILABILITY

In order for your child to begin their educational journey at our facility, the packet below must be completely filled out and returned to our office along with up-to-date shot records. We need all of the below (3) three files filled out and returned.

 For any medication to be administered while your child is in our care, we require the medication permission

 form along with a doctor’s authorization, fully completed. This form must be updated every 7 day
