In order for your child to begin their educational journey at our facility, the packet below must be completely filled out and returned to our office along with up-to-date shot records. We need all of the below (3) three files filled out and returned.

If your child is in need of medication while in our care, we must have the medication permission form below, ALONG WITH THE DOCTORS PERMISSION, filled out in its entirety. This medication form must be completed and updated every 7 days  to be valid
